These systems can be implemented individually or collectively to form a fully integrated community solution providing up to date patient clinical data at the point of care.
The systems can also be integrated with clinical data from GP systems within the local community to provide a holistic single view of the patient journey. This data can then be shared, subject to the appropriate data sharing access agreements, with clinical colleagues in GP practices, acute trusts and social services.
The Graphnet solution is easy to use flexible and extremely intuitive - our electronic filing system for example is a replica of a physical filing cabinet and its contents, and our electronic patient forms mimic paper ones.
